We are willing to provide additional support to families by assisting them with registration. If you need assistance with scheduling the appointment, please contact Stephanie Palomo at 773-890-8020, and you will receive assistance directly with the registration process.
If you do not wish to have your son/daughter participate in the services being provided, it is highly recommended that you have your son/daughter seen by their medical provider. Evidence of these immunizations/health exams must be provided to the school no later than March 28, 2022.
